Okie doke, this kind of deed poll is for over-18's only, under-18's need a different one because they require parental consent. What sucks is that your parents don't necessarily require your consent. You don't need to register this anywhere and it doesn't have to be signed by a solicitor, just any professional person (ie a person with a degree in something) who has known you for a year or more. I'll use the example of Joe Bloggs becoming Jo Bloggs, but you can put in your own names. You'll need software for printing out certificates - I think there's something like that in Windows Office. Lay it out in a format similar to a certificate, only put "deed of change of name" across the top in place of "certificate of achievement" and be sure to make it look nice and official. At the end you'll need to sign it with both your new and old signatures next to both your new and old names, consider it a milestone.


By this deed of change of name made by myself the undersigned Josephine Andrea Bloggs of 12 Example Street, Exampletown, Examplecounty, AB12 3CD formerly known as Joseph Andrew Bloggs.

Hereby declare as follows:

1. I absolutely and entirely renounce, relinquish and abandon the use of my former name of Joseph Andrew Bloggs and assume, adopt and determine to take and use from the date hereof the name of Josephine Andrea Bloggs in substitution for my former name of Joseph Andrew Bloggs.

2. I shall at all times hereafter in all records, deeds, documents and other writings and in all actions and proceedings, as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Josephine Andrea Bloggs as my name, in substitution for my former name of Joseph Andrew Bloggs so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished by the name of Josephine Andrea Bloggs only and not by my former name of Joseph Andrew Bloggs.

3. I authorise and require all persons at all times to designate, describe and address me by the adopted name of Josephine Andrea Bloggs.

Its Josh Edward Surname.

Edward is my Grandfathers name, so its chosen as my middle name, as a tribute to my role model

I'm still deciding on my middle names. It's a toss up between Daniel, Thomas and Sean, or a combination.

I'll probably just end up asking my parents.

Well I think three middle names might be a bit much! But I like I said, my parents will probably get the final decision. They hate the name Nathan, but there's no way I'm dropping that. They knew that if I was born a boy they would have called me Daniel, so I'm assuming they're going to go with that.

Well I changed my surname for family reasons about 4 years ago and I just pretty much went into places like my Doctors to begin with and asked them to acknowledge my new name, showed them the deed poll and that was pretty much it.

I then went onto banks, council (tax reasons) and so on.

I never had a Doctor's note, just my deed poll and birth certificate.

Most Doctor's will charge you for this type of note (usually somewhere around £10) and for something that isn't really needed, I wouldn't bother with it myself.

As soon as you have the little establishments change your name, you can then use letters from them, eg bank statements, to change the details with bigger organisations like the passport office, Inland Revenue and so on

I was purely talking from the name change point of view, not gender.

I haven't a clue when it comes to that gender change on documents, so I can't really comment on that part. :)
